Ebola Reaches an Eighth Congolese Province as Cases Pass 9,400, Even as New Infections Slow

Posted on October 20, 2026 By Carlos now - SI ideas & work

The Ebola outbreak in the Democratic Republic of the Congo has crossed into yet another part of the country. Health authorities confirmed on Monday that the virus has reached Nord-Ubangi, a province in the far north-west on the border with the Central African Republic. It is the eighth Congolese province touched by the epidemic since it began in May, and the news arrives at an awkward moment: national figures had just started to show the first cautious signs of a slowdown.

The provincial governor, Jean-Bosco Botongo, announced two confirmed cases in the Wapinda health zone. One of the two patients, both men in their twenties, has died. The other is being treated at the general referral hospital in Wapinda. Officials have not yet said how the infection reached the area, which lies hundreds of kilometres from the epicentre in the east of the country. That open question is exactly what worries epidemiologists, because an unexplained case usually means there are chains of transmission nobody has traced yet.

Response teams moved quickly. Coordination meetings with technical and financial partners were held in both Wapinda and the provincial capital, Gbadolite, and a local response committee has been set up in the affected health zone. Its first jobs are familiar ones from every Ebola outbreak: following up on alerts, investigating the two cases, finding and monitoring every person who had contact with the patients, and making sure there are isolation beds ready if more cases appear.

The scale of the overall epidemic is sobering. According to figures released over the weekend, the country has recorded 9,455 confirmed cases and 4,561 deaths, a fatality rate of around 47 percent. Some 925 patients are currently being treated in isolation, while 2,530 people have recovered. The outbreak now touches 66 of the country’s 178 health zones. Between the middle of last week and Friday alone, the European Centre for Disease Prevention and Control counted 178 new confirmed cases and 83 additional deaths.

The burden is spread very unevenly. Ituri province in the north-east remains the heart of the outbreak, with 6,918 cases and 3,197 deaths reported from 28 of its 36 health zones. Neighbouring Nord-Kivu has recorded 2,036 cases and 1,156 deaths. Haut-Uélé follows with 408 cases, then Tshopo with 73 and Bas-Uélé with 13. Sud-Kivu, Sud-Ubangi and now Nord-Ubangi have each reported only a handful of cases so far. The challenge for health workers is to keep those small numbers small.

Signs of hope, and a strain without a licensed vaccine

Despite the geographic spread, the latest national situation report struck a cautiously optimistic tone. On 16 October the country recorded 67 new confirmed cases, compared with 71 the day before and 69 a week earlier. Deaths in the community, which often signal that sick people are not reaching treatment centres in time, fell to 18 from 30 a week before. Deaths inside treatment centres also dropped. Twelve health zones have now gone at least 42 days without a new case, the threshold used to declare that transmission has ended locally, and several more are getting close.

Those numbers suggest that the core of the response is working in many areas. Contact tracing reaches around 80 percent of known contacts, a figure that is far from perfect but solid for a region with poor roads, insecurity and large population movements. Every recovered patient discharged after two negative tests is also an important message to communities that treatment centres save lives, which helps persuade people with symptoms to come forward early.

Yet this outbreak is harder to fight than some earlier ones. It is caused by the Bundibugyo species of the Ebola virus rather than the more familiar Zaire strain. The licensed vaccine that helped end previous Congolese outbreaks was developed against the Zaire strain, so responders here cannot lean on mass vaccination of contacts in the same way. That puts more weight on the classic tools: fast detection, isolation, careful care for patients, safe and dignified burials, and constant engagement with communities.

Trust is a crucial part of that work. In past epidemics, rumours and fear of treatment centres led families to hide sick relatives or to hold traditional funerals that spread the virus further. Local health workers, religious leaders and community radio stations play a big role in explaining what Ebola is, how it spreads through contact with bodily fluids, and why early treatment improves the chances of survival. The arrival of the virus in a new province means that this work has to start again from scratch in communities that may never have seen an Ebola response before.

The location of Nord-Ubangi adds a regional dimension. The province borders the Central African Republic, and river traffic and trade link it with neighbouring countries. Health officials in the region will be watching closely for any cases that cross the border, and international partners are likely to push for stronger screening at crossing points and along the Ubangi river.

For now, the picture is mixed. Nationally, the curve seems to be bending slowly in the right direction, with fewer deaths and more zones reaching the 42-day mark. Locally, the jump to a new province shows how quickly the virus can find gaps in surveillance. Whether the outbreak keeps shrinking will depend on how fast the teams in Wapinda can trace the origin of the two new cases and close that chain before it grows.

The coming two to three weeks will be telling. Ebola has an incubation period of up to 21 days, which means any further infections linked to the new cases should become visible within that window. If Nord-Ubangi stays at two cases, it will be a sign that the rapid response worked. If more appear, the region could become a new front in an epidemic that has already claimed more than 4,500 lives.
